iphone application.

This is a pretty simple iphone application. This is database driven and has around 19 pages. The user can input information into text fields, create text information, select from a combo box, and then access this information on different screens. This app would also allow the user to take an image on one of the pages and add it to an information page. This iphone app. will also have a calendar option with certain text fields populating the calendar. This iphone app will also have a page that allows the user to store phone numbers and pull phone numbers from the users address book, and store them within this application, with the ability to share the phone numbers to another users same application. This will need to be designed as well, and am open to suggestions, though will probably use a lot of the core buttons and page functionality for this app. It doesn't have to be 'super fancy.' If you need me to design this application, I certainly can, though you need to let me know what type of file format you need. I generally use Adobe Illustrator when I design.
