Based on a spreadsheet with data from 700 users, containing code, name, password, and group, I need a PHP system that show a screen with a login and password box. The user must enter their details and they will be validated according to the spreadsheet (code and password). If the data is valid, then users can respond to a poll of nine items of simple choice. All data and replies shall be stored in a MySql database. There must be a master password with access to a routine that issues a report stating the percentage of each group who have completed the survey. Each user can only complete the poll once, and then their access must be blocked. Login
