The attached pdfs contain scans of a standard form which is filled in by hand. The pdf is a handwritten standard form. Each page is the same and the fields in the form need typed into the spreadsheet - the names of the boxes on the form correspond to the column heading on each column. Each row in the spreadsheet should be one page from the pdf.
