Survey Style Mobile Landing Page (smartphones)

1. I need a template for a 4 step "survey style" smartphone landing page that works for smartphones (iphone, android, samsung s2, note etc) 2. see example here - [login to view URL] - this is the same format, layout, I want - when user clicks on yes / no, it takes them to next step - when user enter answer into text field, it takes them to next step. 3. Must load super fast by combining external JavaScript, optimizing images and leveraging browser caching if necessary. 4. Must be easy to edit. I may need to create different survey style questions. I should be able to edit text, and add different phone number - I have basic knowledge of html and use dreamweaver mx, - coder must provide clear instructions, video, images etc on how to edit files - coder must also provide clear instructions on uploading to my server (show video if necessary) 5. Must be thumb-friendly with vertical scrolling if needed 6. Content fits onscreen and can be read without pinching and zooming. 7.
