AJAX Shows Form, When MySQL Entry Changes

Hi there, I need an AJAX page that creates a new entry in a MySQL table* and makes a simple CURL request. When the row (that was just created) `confirmed` value becomes 1 (previously 0), I want a form to display on the page. *This row in MySQL is dynamically inserted when the page is fetched... so for example if 124 is the auto_increment value available for the next entry... we would insert (124,data,data,0)... then every 2 seconds we check the database to see if it becomes (124,data,data,1)... and when it does it shows a form. Simple. My budget is ~$50. :) Thanks
