Write an agreement to my models, giving me their permission of using their photos/videos for commercial use

I am about to start up a 3D photography business in the MA, US - which is similar to normal photography studio, but instead of providing customers photos, I make 3D-Print scaled figures of real people for them to bring home in the end. I am thinking of inviting some people to come over and become "models" to test my 3D capturing system. I will take photos/videos while they visiting, and then planning to use these materials in my product promo video/brochures/website etc. in the next couple months. So I was wondering in this case, can you suggest which kind of agreement my "models" should sign to let me use their video/photos for my product promo? Can you help to write it? Thank you in advance!
