Create 2 Forms using supplied Content

1) I need the QPSL Cr. Application Draft to be like the A N S A Coaitings Form Attached. So where someone must input information in using the form, there would be underlines not boxes, we will need to use as well the seperators, Check boxes, etc. as was done in the Penta Form. 2) I also need the Warranty Form dealt with similary building on the format used in the Draft, making it a in sync with the theme/layout of the ANSA Coatings Form as possible. I would want the 2nd one done first as it is simpler. Then we will move on the the First one.
