What I want is a PHP file which includes: - A form - Form validation - Captcha Protection --A form Here are the form fields: - First Name (text) - Last Name (text) - Address (text) - Zip Code (text) - Country (Drop down list) - Email (Email) - Telephone (text) - Notesa (text-area) - Notesb (text-area) - Terms of Service (check box) - Submit button When a user clicks the submit button the form is submitted to my e-mail address.. The e-mail message includes all the information of the form.. ATTENTION! The sending of e-mails must be done so as to use utf-8 encoding (Unicode) both in the subject of e-mail and e-mail message.. --Form validation All fields are required.. The form must only accept a valid email.. --Captcha Protection Use a nice captcha image to protect from spam..
