Customer edit page fix (oscommerce)

We use oscommerce and are having a couple of problems with the customer edit page on the admin side of things. When we view the customer edit page almost all customers' countries say "Afghanistan" instead of their actual country. And when we try to save any edits that are made we keep on being prompted to type in the state/province, even when there is already something in there. Please see the attached screenshots. All of your work must be well commented and we'll need you to write a report detailing any database and page changes and/or additions.
