database update through web page checkboxes

I want to update/insert records into mysql database. I have a form with some inputs text, and a check box. The form is within a repeater, so depending on the number of records read from the database, the number of forms produced. What I want is, for the forms whose checkbox is checked I would like to save the fields items to a database record when the submit button is clicked. I will include a page sample to give you and idea of what I have in mind, could be done in javascript/php, but prefer javascript. ## Deliverables I want to update/insert records into mysql database. Some things to note: You probably have to dynamically set the check box id and value properties.
