Simple Form Data Post

Hello, I need a form script that is very secure (no mysql injections). The form needs to be evenly broken up into 3-4 pages. (please review attached image) You can find what form fields are needed by the posting instructions here [login to view URL] When a user submits their data, the data will then be submitted to my database and the posting instructions given above. When you look on the posting instructions you will see that it allows you to post the data up to 3 times consecutively at different prices. I would like to be able to do this and set the 3 different prices within the script. Create the database on your end and then I can just change the info for my database once the project has been completed. Please look at the attached image for detailed instructions for data flow.
