PHP Coder for 4-5 hour project to start now(repost)

I am working on the order form [login to view URL] and all I'd like done is: 1) Modify the header, footer and background to fit the site [login to view URL] (the header is almost done, but needs to show the right text at top) . Order information (description, price, etc.) should be organized nicely at top in a horizontal box. 2) Pass the coupon code and valid domain name to the page instead of having to enter in the page 3) If the product price is $0.00, don't show the paypal or [login to view URL] part (let person check out for free since it's free). The code is open source and very nicely organized. However, I need to finish some other things at the moment.
