Vitelity.com Search Clone - Asterisk Project

What I need is an search web page that works like this [login to view URL] At this URL and towards the bottom, you'll see the words "Local DID Coverage" and below that two search boxes. This is the part I need to clone. In order to use Vitelity's API you'll need to provide an IP. I'll grant access to your IP when the the project is awarded or I can let you use our server.
