WordPress RPG Character Sheet Plugin

I need a forms builder plugin created for WordPress. It is for LARP and tabletop ("pen and paper") roleplaying games. You may adapt pre-existing form plugins for the purpose. If you do that, I will need to know which plugin you are basing the project on. I will also need to know if you are creating a standalone plugin or if you will be making it as an add on. PHASE 1 Requirements: Multiple forms must be able to be created. Created forms must be exportable and importable through the plugin. All form data must be saved to the database. Back end: Forms are built by designating fields (boxes in the form) Fields are named Items are dragged into the fields (Single Line Text, Text Box) Items variables can be dragged onto items, up to 3 of them (Plain Text, Number, Pips, Plus/Minus Number) Items can be designated as fill in the blank or a drop down selection. Database hierarchy and associations: Form -> Field -> Item -> Item Variable Additional details: All submitted forms must be saved and accessible from the back end. Submissions need to be editable by the administrator New submissions should be flagged for approval The submission should be viewable in the front end by the creator (and no other end users). PHASE 2 will mainly consist of setting up a two-part points system. The first will be used up in the form submission. Points can be set by field and item. Universal points can also be added to the form, with configurable costs for items in certain fields, specific types of item variables in field, etc. The other part is a separate universal points field that can have points added by the administrator. When this second pool of points is added to by the administrator, the end user receives an email notification and the form is editable by the user. They cannot change what is already there, but will be able to spend those points to add more to the form. Also, a different level of privacy granularity should be added. The admin can designate any field, item, or item variable as publicly viewable. There will be further development, but those are the two stages I need done immediately.
