Build a multistep form with some conditional logic

Hi, We need a simple website developed to test a sign-up funnel for our product. I am not sure what technology is best, but the site does need some logic. Screen 1 will ask the users a few basic questions. These answers need to be stored somewhere. Screen 2 will show the user some information, based on their answers to the first questions. Screen 3 will show them some (fixed) information about our product. Screen 4 will be a signup screen. All their answers need to be stored so that we can access them. A basic spreadsheet is fine. This whole funnel is a test, and if successful we will then integrate into our main product. This is a mobile-first site, so I have designed it in mobile, but we also need a desktop version, which needs to be fully responsive. We would like it so that when you select an option, it scrolls down to the next question. We need events setup up, so that we can track progress down the funnel in Google Analytics.
