Simple Wordpress website that accepts inputs, performs a calculation and displays results

I need a simple Wordpress website. It will be a dumbed-down version of an app that is on the iTunes store that calculates child support in Texas. The home page will have some text and some inputs. It will then have a button that says “Calculate.” When the button is pressed, a calculation is made and the results are displayed on that page. There will be some more pages, but I can do them myself. It’s the integration of the calculations into the website that I don’t know how to do. I have the calculations coded on a php file that I will give you. You would also load the website for me. The project must be finished by Sunday Dec. 31 at 5:00 p.m. CST which is 11:00 p.m. GMT.
