I need a web and mobile form for gathering pricing source data

In order to create a price for customers I need to collect a lot of data. Today that is done via Excel and I want to do it via the web and an app instead. The Excel document has all the fields required however I would also like to add some features. 1. an information pop up after each field so the user can get self help 2. required and pre populated fields tied to the user login 3. Alerts letting the next person in the process know work is in queue
