The difference between paper and computer would of course be that you cannot have circles drawn over the answers - rather it would be either option buttons or a list/combo box where the user makes his choice. Project: Create a form and a report Build the form using the supplied image as a guide (this is not a "paste", can't do that with an image) Attach the fields in the form to the table(s) Create a report based on the image. (Make a report that looks like the image) Test everything. This is a bit of work but can be done without any tricks. Good Afternoon, After reveiwing the attached image and considering what it is you are requesting it seems that what we could do to provide you with the solution your are looking for is. Create a form for editing finished paperwork and future paperwork, this would store all the information in a single table, each record being equivalent to each paper, Then create a report which is styled exactly like your Paper for future viewing and printing, this would give you the opportunity to be able to look up reports based on name, and have the capability to print in case for some reason the previous document had been misplaced. I have been coding and transcribing paper capture forms to databases for the last eleven years. Once a system specification document has been agreed to for the database and capture process to implement it will be quick and easy. Not forgetting the validation process to make sure the database is a true and accurate copy of the paper based CRF's. This can be accomplished with SQL and C# or in Access as per the clients request.
