I need my website corrected: [login to view URL] 1. Remove “1” on line ethnicity. 2. Add a section where person filling out application can upload up to four (4) pics form their hard drive. (For an example see: [login to view URL] ) 3. Make the number that is generated at the bottom of the application a “progressive” number – currently it is a “random” number. 4. Add a section where person filling out the application has to specify “male” or “female” – check boxes preferred. [login to view URL] 1. Remove the line that reads: “Please click here to upload your picture”. [login to view URL] 1. I need the ability to change the user name and or password. 2. I don't want the username and password to automatically be filled in (as it is now) – I would like them to have to be entered each time we want access. 3. I would like the ability to add multiple users with unique usernames and passwords. Lastly, I would like a banner created from the logo on the main page that I can use as an “ad banner”
