Convert the form at [login to view URL] to be accessible. - Must pass Siteimprove Accessibility Checker (Chrome extension) at level AAA - Must pass WAVE ([login to view URL]) with no errors and not alerts - Uses Bootstrap 4.6.2 - All css changes should be added to html in the header - Radio buttons and checkboxs - If they all fit on one line then display in-line - If they do not all fit on one line then display in columns - If any of the text wraps to a second line while displayed in multiple columns then display in one column - If text extends to multiple lines while displayed in one column then the additional lines are indented under the text instead of under the radio button (like an <ul> <li> does) - Nested html elements indented at 1 space per level – like the <head> element is - All elements with ids should have a comment at the start and end, like the “survey” id - All fieldset elements should have a comment at the start and end, like the “survey” id I am open to any other visual/accessible design recommendations that you may have.
