Order Form Conversion

4 Printed page order form needs converted to php or html with a database or data structure (xml, mysql, etc.) to support holding the form items that will indicate inventory for selection from within the form. For example, under the field "Product," there would need to be a combo box or some mechanism that when the letter D is typed, that the product list goes to all products beginning with D. Once all selections are made and the form submitted, just a basic print out of the form with the field selections and notes is required.
