I am available to get started on creating a mobile HTML5 based app that works on iOS, Android, blackberry, windows phones and tablets. The only thing I need to get started is more information on how you want it to work. I already understand the basic functionality. As the app will not be complied for each platform but run on the web it does not have native access to the phone so things like the camera can not be accessed directly through the app. Which I think might make the whole process more streamlined but if you can settle with taking the pictures with the device camera app and then uploading them through the form this is not a problem. The process will go something like this: First we discuss how the app will behave and once we are on the same page I will create the design/shell for the app which will consist of everything but the logic that powers the app. Once you are satisfied with this I will create the logic that handles form input and upload. The app will have an UI native to the device it is viewed on but carry an identical layout across all platforms. So it will basically look and feel just like an app only it is run on the web. Because the will be built on HTML5 technologies it will be very easy to manage and if you wish even compiled and published as proper apps for each platform using something like PhoneGap. Let me know if you are interested so we can discuss things further. Also I require a 35% payment upfront. The rest upon completion.

Hello, I just created the same type of app for an independent contractor in Alabama (Catastrophe Services) that is doing Flood work for the government. Their app is NFIP Forms2Go found on the iPad. The app you describe is fairly easily, but the form portion is what is time consuming. I wish I could do it within your budget, but I know how long the Flood project took. Anyways, I am in Ft. Although his app was for the iPad, I will soon be doing an Android version of his app too (he wanted to start with the iPad, but I gave him the option of starting with either).
