.net 2.0 question form

We need a .net form which is divided into two parts: 1 pure html file ( ONLY html code..this is due to server restrictions ). which is the question form This file will contain an action attribute which points to the asp.net file which is to process the input of the user. Question form should contain 3 questions. Each question contains 3 radiobuttons with answers the users can choose 1 from each question. Should also contain an input field for the submitters email adress (javascript validation to check that a real email is entered). Once submittted the form, the answers should be sent to one of our emails containing what the user has answered and his email adress. Once submitted, a "thank you" message should be displayed inside the html document to the user.
