5 Contact forms creation and simple php+mysql actions

I need you to create 5 contact forms starting from my html drafts. Forms will have all the same fields but will be in different languages as labels. I provide you the basic html forms with no action, requested field check, date field. These 5 basic forms will be in English, French, Italian, German and Czech. Fields names can be always the same, the localized labels changes and I'll provide those in my staring files. Forms should: - have a popup DATE selection field, working with major browsers - check the required fields (better if using ajax+jquery) highlighted with a * - have the possibility to set a "thank you" page urls (different for every form of 5) - have the possibility to config a mysql access (same for every form) - be accessible and better if table less - have a web2 look (highlight of selected fiels, help text etc) also using external css - better is be possible to integrate in a page, using php include. Or as stand alone php file Action should: - send the fields as formatted email, to our addresses (2 emails) - send a copy to the user email - save the results in one unique (all forms results go in one db) mysql database. You'll have to send me the sql dump to create it.
