Create Wordpress Plugin (Sepsis)

I want to buy dynamic form for Word Press. It will assist in a special task: coding septicemia in the International Classification of Diseases (ICD). This is rather challenging in Germany: many rules have to be obeyed. The form asks a few questions after which it comments on the possibility of coding septicemia. I need this form as plug-in. It should be easy to use and easy to service: Every year the codes and coding rules are revised, which may influence the coding of septicemia too. _Before bidding_: please read all the requirements and the details of the business rules in the attached file first. ## Deliverables See the specs in the word-document! The product must comply with the following rules and conventions: * Plugin for Wordpress 3 and higher for installation as ZIP-file. * Plugin complies with Wordpress Plugin conventions * Plugin uses the Wordpress PHP interface / API where applicable * Where the WP API is not used, the plugin is programmed independently * If the worker wants to use/merge other plugins please consult with me first * The form must be handled by a Wordpress shortcode ([form-sepsis]?) * Form output must follow the HTML5 standard * The use-case information is found in the attached file We use the canvas theme: the Woo Framework can be used accordingly This will not be the last plugin I need for my site. At least three more are to come. I'm looking for a long-term business relationship. I'd very much appreciate if we could work along the following lines: * You quote a price you think appropriate for the work as it is explained in detail in this project description. * While working: If you think I'm changing the specs or requiring extra features that go beyond the scope of your bid, you should tell me about it right away. We'll sort things out immediately. If necessary we'll fix a price for the extra's (as a bonus). * I will not negotiate about giving a bonus "after the show". Often workers ask for a bonus after the work is accepted like: "I've worked hard and it wasn't easy so if you liked my work please pay a bonus of 10% or more". Let's negotiate a price that fully satisfies you and that you think is fair _in advance_ instead.
