Form Creation - Orgenize collected Datas in Table

Hello, I have created a Registration Form with HTML and CSS. It has already some Fields created by me. but it still don't work the way I want it to. In fact it misses some new Fields & Features I need to : Add some new Fields to the Form. Organize the form's Data in a table. ------------------------------------------------------ New Fields ---------------------------------------------------------------- - 1st New Field : Domain Name Availability Checking : This field is divided into two fields, First one is the DomainName, Second Field is for the Extensions. For example : [ DomainName ] - [ com ] ( and it should check for all existing extensions ) The form automatically checks if the Domain is Available or not. without Refreshing the Page. If available : a message telling : " the Domain is Available " is shown in Green. if NOT available : a message telling : " the Domain is not Available " is shown in Red. - 2nd New Field : Annual Pricing: Than comes a Drop Down List field, that I have created. It has all the available Subscription Plans of our site, with prices Set per Month. User will select a Plan, then automatically, the annual price is shown, right before the Captcha & SUBMIT button. That Annual Price Field, should appear only when a Plan is Selected. It has to show two prices : • (12* price / month , without VAT ) in Bold, and big font, (approx 16px) Black colored • (12* price / month + VAT ) in normal font, (approx 12px) grey colored - 3rd New Field : Captcha Field: Right before the SUBMIT button, it has to be a simple image captcha. - 4th : Check Box: if users don't want to receive newsletters, they will have to check that CheckBox. ------------------------------------------------------ Organize Form's Data ---------------------------------------------------------------- I want form's Data to be organized in a table that can be broken into : 20 - 50 - 100, lines : Table must always show recent entries first. An "Archive Button" has to be placed besides each entry, to send it to an "Archive Table". Table's Data can be extracted into Excel format Filter Data, take off the user that didn't subscribe for the NewsLetter. ------------------------------------------------------------------------------------------------------------------------------------------------- Other projects are on track and they will require some more customization, this is why we are looking for a serious reliable coder, to get going with us for further works. Not to mention that the Java Script, HTML and CSS, has to be clean / neat.
