Registration Module for Website - repost

Hello. We have to create a registration module for our website. It would be a guided registration process like Linkedin. Have mentioned the details in the attached document. The person has to make the entire HTML, CSS, JSS and PHP. However, the project needs to be completed by 24th October, 2013 - 11:59 PM [Indian Standard Time]. Time is the essence of the project.
