Submission Form Date Calendar Popup

We have a standard submission form which lets the user input a desired date and e-mail it to us via PHP. The feature that we'd like this form to have is a calendar beside the date fields. If the user clicks on this "pop up calendar" they are able to select a date and have it auto update said date fields. The date fields should also automatically to the current date, so if "today" is the desired date, no changes must be made and can just be submitted. (Please note we already have the form, just looking for the added calendar feature) This feature is used on many if not most websites, all we require different than those are customized color and look. This is a rush project being it's tiny nature, and please note we will not be responding to all bid inquiries we receive. Thank you.
