479353 Need help customizing automated email php script

Attached is the code I am currently using. This code is used in an education gradebook system by teachers and students. Some students are required to take a yearly quiz. The script looks in the mySQL DB for the grade, looks at the date it was inserted, and if it was 351 days ago (line 4), it sends out an email to the professor for that course alerting them. The alert is generated on line 57, and prints the course name, and then any students name below. The code works perfectly. The change I need is this: If the script detects a the quiz name of 'Basic Haz Mat Training', it should only be included in the email if the last grade is three years old, instead of one. This quiz is only required every three years, so we do not want to include the quiz in the outgoing email if it hasn't yet been three years. I am sure this is a simple change, but I am not a proficient enough programmer.
