Prize website - Co-Registration

Hi Freelancers, We need the following: A website where on the first page information about the user is being collected (name address, email etc.) And then on the 2nd and 3rd page questions are answered Example do you like to win free prizes with the answers Yes or No If answer is Yes, then either a URL string has to be called - with the data of page 1, or a dump to an Database/excel sheets to be made. All the data needs to be stored into an excel sheet or SQL database containing all the info of the user and all the answers on the questions. That's basicly it - web design is not really neccesary so it's more or less the code and a working demo page Good luck bidding!
