573698 Implement digital signature code and generate PDF

We need to record a digital signature from our users before payment stating that they agree to our terms. This then needs to be saved into a PDF file on the server. For the digital signature this code looks suitable - [login to view URL] What we would like is a lightbox style webpage to be shown to the user when they click the checkout button. This will have the terms and conditions listed and at the bottom of the page, the above signature feature. Once signed and submitted, the terms, date, full name, signature and Computer setup/IP adrress will be saved into a PDF file on the server. The server is running PHP with custom coded website.
